Louisiana Women’s Healthcare is committed to protecting your privacy and to giving you timely, accurate access to your medical records when you need them.

Louisiana Women’s Healthcare partners with MRO to process medical records requests.

Important Additional Notes

  • All requesters must provide proof of identification or documented proof of legal relationship – personal representative, patient advocate, executor, trustee or guardian. Those with a legal relationship with the patient as listed will need to provide proof of identification.
  • Medical records requests are processed in the order they are received and will be delivered to you as quickly as possible.
  • Depending on the purpose of your request, there may be a fee for a copy of the records. You will be advised if there is a fee when your request is submitted and again before it is completed.

New Patients

If you are a first-time patient of Louisiana Women’s Healthcare, please follow the instructions below to schedule an appointment with your preferred LWH physician.

  • To search for a specific LWH physician and/or preferred appointment date, click “Filters” or “Edit” to further customize your search.
